ABSTRACT

Introducción: La región glútea ha sido considerada una de las zonas más observadas tanto en mujeres como en hombres y constituye un ícono representativo de belleza corporal. En los últimos años se ha incrementado el deseo de las féminas de aumentar esta región anatómica con implantes. Objetivo: Determinar los resultados estéticos de la gluteoplastia de aumento con implantes intramusculares. Métodos: Se realizó un estudio prospectivo, longitudinal y descriptivo de 25 mujeres que acudieron a la consulta externa del Servicio de Cirugía Plástica del Hospital Clínico-Quirúrgico Hermanos Ameijeiras, desde agosto de 2016 hasta febrero de 2019, para solicitar el aumento de su región glútea con implantes. Resultados: En la serie predominaron el grupo etario de 31 a 40 años (36,0 %) y el fototipo de piel III (88,0 %); asimismo, solo 6 pacientes presentaron complicaciones (24,0 %), entre las cuales sobresalieron el seroma (20,8 %), la asimetría (12,5 %) y la dehiscencia de la sutura (8,3 %). Se observó un aumento significativo de la proyección glútea después de la intervención (α=0,05) y los resultados fueron buenos en la mayoría de las integrantes de la muestra (84,0 %). Conclusiones: La gluteoplastia de aumento con implantes intramusculares es un procedimiento seguro y ofrece resultados estéticos favorables.


Introduction: Gluteal region has been considered one of the most observed areas either in women as in men and constitutes a representative icon of corporal beauty. In the last years the desire of women to increase this anatomical region with implants has been increased. Objective: To determine the aesthetic results of the augmentation gluteoplasty with intramuscular implants. Methods: A prospective, longitudinal and descriptive study of 25 women that went to the Plastic Surgery Service outpatient consultation of Hermanos Ameijeiras Clinical Surgical Hospital was carried out from August, 2016 to February, 2019, to request the increase of the gluteal region with implants. Results: In the series there was a prevalence of the 31 to 40 age group (36.0%) and the skin phototype III (88.0%); also only 6 patients presented complications (24.0%), among which seroma (20.8%), asymmetry (12.5%) and anastomotic dehiscence (8.3%) were notable. A significant increase of the gluteal projection after the intervention was observed (α =0.05) and the results were good in most of the members of the sample (84.0%). Conclusions: The augmentation gluteoplasty with intramuscular implants is a safe procedure and offers favorable aesthetic results.

ABSTRACT

Abstract Intramuscular hemangiomas (IHs) are benign soft-tissue tumors that represent less than 1% of all hemangiomas. This clinical entity is rarely considered as a differential diagnosis in cases of musculoskeletal pain. A healthy 38-year-old woman presented to our office with complaint of left omalgia, with 8 months of evolution, limiting her daily activities. She reported the appearance of tumefaction in the previous 4 months. She was medicated with analgesic and antiinflammatory drugs with no clinical improvement. The objective examination showed limitation of left shoulder abduction (0-90°). The patient underwent a magnetic resonance imaging (MRI), in which a well-circumscribed nodular formation was detected in the deltoid muscle. Then, she underwent a biopsy, which confirmed the diagnosis of hemangioma. The patient was referred for sclerotherapy. Intramuscular hemangiomas are usually observed in young patients. The gold-standard examination for diagnosis is MRI, which often forestalls the need for a biopsy. In many cases, IHs are asymptomatic and tend to involute over time. Despite the low frequency of this clinical entity, it is important to place it as a diagnostic hypothesis in cases of chronic pain of the limbs in young patients with poor therapeutic response to antiinflammatory drugs and analgesia.


Resumo Os hemangiomas intramusculares (HIs) são tumores benignos de tecidos moles que representam menos de 1% de todos os hemangiomas. Esta entidade clínica raramente é considerada como diagnóstico diferencial nos casos de dor musculoesquelética. Uma paciente do sexo feminino, de 38 anos de idade, saudável, se apresentou ao nosso consultório com queixa de omalgia esquerda, com 8 meses de evolução, que limitava suas atividades diárias. Ela relatou o aparecimento de tumefação 4 meses antes da consulta. A paciente estava medicada com analgésico e antiinflamatório sem melhoria clínica. Ao exame objetivo, ela apresentava limitação da abdução do ombro esquerdo (0-90°). A paciente foi submetida a uma ressonância nuclear magnética (RNM) na qual foi detectada uma formação nodular bem circunscrita no músculo deltoide,. Em seguida, foi realizada uma biópsia que confirmou o diagnóstico de hemangioma. A paciente foi então encaminhada para a realização de escleroterapia. Os HIs normalmente são observados em pacientes jovens. O exame padrão-ouro para o diagnóstico é a RNM, que muitas vezes torna a realização de biópsia desnecessária. Em muitos casos, os HIs são assintomáticos e tendem a involuir com o tempo. Apesar da baixa frequência desta entidade clínica, é importante colocá-la como hipótese de diagnóstico em casos de dor crônica dos membros em pacientes jovens com má resposta terapêutica a antiinflamatórios e analgesia.

ABSTRACT

Human cysticercosis is caused by cysticercus cellulosae, larvae of a tapeworm, taenia solium. Cysticercois can involve any tissue in the body even it has maximum tendency towards neural tissue. The most common affected sites are central nervous system, subcutaneous tissue, eyes and muscles. An isolated case of intramuscular cysticercosis is a rare presentation. Here we present the case of a 35 year old female with a swelling on the posterolateral aspect of right arm and was diagnosed as myocysticercosis which was abuting to the extensor muscle that is triceps muscle on high resolution ultrasound and ct scan and it was managed by antihelminthic medications followed by surgical excision.

SUMMARY OBJECTIVE: The aim of this study was to compare the levels of pain developed during intramuscular injections to the laterofemoral and ventrogluteal regions in children. METHODS: The study population consisted of all children aged between 7 and 12 years who presented to the pediatric emergency clinic of a hospital. The sample consisted of 62 children who met the inclusion criteria and agreed to participate in the study, and the children were randomly assigned to each group (laterofemoral n=31, ventrogluteal n=31). "Buzzy" and "deep breathing" were applied to children in both groups to relieve pain during the procedure. The data were obtained using an Information Form, a visual analog scale, and the Facial Pain Scale-Revised. RESULTS: It was determined that the children in the ventrogluteal group during the intramuscular injections had lower visual analog scale and faces pain scale-revised scores immediately after the procedure compared with the vastus lateralis group, that is, they experienced less pain, and the difference between the two groups was significant (p<0.001). CONCLUSION: In children, it is recommended to choose the less painful ventrogluteal region for intramuscular injection and to inform health professionals about it.

ABSTRACT

Esfenvalerate is a kind of commonly used highly effective pyrethroid insecticide. It is common for people who are poisoned by contact or misuse, but rarely reported for people who are poisoned by intramuscular injection. This paper reports a case of intramuscular injection of esfenvalerate in the Department of Infection, West China Hospital of Sichuan University in November 2021. The patient was intramuscularly injected with about 20 ml of esfenvalerate, inducing the sense of swelling and tingling, degeneration and necrosis of striated muscle tissue at the injection site, also liver function damage and other manifestations. The patient was discharged from hospital after rehydration, accelerating poison metabolism, anti-infection, liver protection and local puncture.

ABSTRACT

The aim of this study was to clone the goat RPL29 gene and analyze its effect on lipogenesis in intramuscular adipocytes. Using Jianzhou big-eared goats as the object, the goat RPL29 gene was cloned by reverse transcription-polymerase chain reaction (RT-PCR), the gene structure and expressed protein sequence were analyzed by bioinformatics, and the mRNA expression levels of RPL29 in various tissues and different differentiation stages of intramuscular adipocytes of goats were detected by quantitative real-time PCR (qRT-PCR). The RPL29 overexpression vector pEGFP-N1-RPL29 constructed by gene recombination was used to transfect into goat intramuscular preadipocytes and induce differentiation. Subsequently, the effect of overexpression of RPL29 on fat droplet accumulation was revealed morphologically by oil red O and Bodipy staining, and changes in the expression levels of genes related to lipid metabolism were detected by qRT-PCR. The results showed that the length of the goat RPL29 was 507 bp, including a coding sequence (CDS) region of 471 bp which encodes 156 amino acid residues. It is a positively charged and stable hydrophilic protein mainly distributed in the nucleus of cells. Tissue expression profiling showed that the expression level of this gene was much higher in subcutaneous adipose tissue and inter-abdominal adipose tissue of goats than in other tissues (P < 0.05). The temporal expression profile showed that the gene was expressed at the highest level at 84 h of differentiation in goat intramuscular adipocytes, which was highly significantly higher than that in the undifferentiated period (P < 0.01). Overexpression of RPL29 promoted lipid accumulation in intramuscular adipocytes, and the optical density values of oil red O staining were significantly increased (P < 0.05). In addition, overexpression of RPL29 was followed by a highly significant increase in ATGL and ACC gene expression (P < 0.01) and a significant increase in FASN gene expression (P < 0.05). In conclusion, the goat RPL29 may promote intra-muscular adipocyte deposition in goats by up-regulating FASN, ACC and ATGL.

ABSTRACT

The purpose of this study was to clone and characterize the ZFP36L1 (zinc finger protein 36-like 1) gene, clarify its expression characteristics, and elucidate its expression patterns in different tissues of goats. Samples of 15 tissues from Jianzhou big-eared goats, including heart, liver, spleen, lung and kidney were collected. Goat ZFP36L1 gene was amplified by reverse transcription-polymerase chain reaction (RT-PCR), then the gene and protein sequence were analyzed by online tools. Quantitative real-time polymerase chain reaction (qPCR) was used to detect the expression level of ZFP36L1 in intramuscular preadipocytes in different tissues and adipocytes of goat at different differentiation stages. The results showed that the length of ZFR36L1 gene was 1 224 bp, and the coding sequence (CDS) region was 1 017 bp, encoding 338 amino acids, which was a non-secretory unstable protein mainly located in nucleus and cytoplasm. Tissue expression profile showed that ZFP36L1 gene was expressed in all selected tissues. In visceral tissues, the small intestine showed the highest expression level (P < 0.01). In muscle tissue, the highest expression level was presented in longissimus dorsi muscle (P < 0.01), whereas the expression level in subcutaneous adipose tissue was significantly higher than that in other tissues (P < 0.01). The results of induced differentiation showed that the expression of this gene was up-regulated during adipogenic differentiation of intramuscular precursor adipocytes (P < 0.01). These data may help to clarify the biological function of the ZFP36L1 gene in goat.

ABSTRACT Objective: to validate a Standard Operating Procedure on the intramuscular vaccine administration technique in adults using high frequency vibration associated with cryotherapy. Methods: a literature review on intramuscular vaccination practice using a vibration device associated with cryotherapy. Then, a form was created to validate the instrument, detailing the items that were assessed by judges following recommendations in the literature. Judges' answers were assessed using the Content Validity Index, with items whose index was greater than or equal to 0.80 being validated. Results: twenty-five nurses participated in validity, identifying judges' opinion regarding item relevance, clarity and accuracy. Judges validated the instrument, according to the values that remained between 0.88 and 1.0. Conclusions: the instrument developed and validated is a tool capable of guaranteeing safety and standardizing immunization practice in vaccine rooms.


RESUMEN Objetivo: validar un Procedimiento Operativo Estándar sobre la técnica de administración de vacunas intramusculares en adultos mediante vibración de alta frecuencia asociada a crioterapia. Métodos: revisión de la literatura sobre la práctica de la vacunación intramuscular mediante un dispositivo de vibración asociado a la crioterapia. Luego, se elaboró un formulario para validar el instrumento, detallando los ítems que fueron evaluados por los jueces siguiendo las recomendaciones de la literatura. Las respuestas de los jueces fueron evaluadas mediante el Índice de Validez de Contenido, siendo validados los ítems cuyo índice fuera mayor o igual a 0,80. Resultados: 25 enfermeros participaron de la validación, captando la opinión de los jueces sobre la pertinencia, claridad y precisión de los ítems. Los jueces validaron el instrumento según los valores, que se mantuvieron entre 0,88 y 1,0. Conclusiones: el instrumento desarrollado y validado es una herramienta capaz de garantizar la seguridad y estandarizar la práctica de inmunización en salas de vacunas.

Métodos: revisão de literatura sobre a prática de vacinação intramuscular utilizando dispositivo de vibração associado à crioterapia. Em seguida, elaborou-se um formulário para validação do instrumento, com detalhamento dos itens que foram avaliados por juízes seguindo recomendações da literatura. As respostas dos juízes foram avaliadas pelo Índice de Validade de Conteúdo, sendo validados os itens cujo índice foi maior ou igual a 0,80. Resultados: 25 enfermeiros participaram da validação, captando o parecer dos juízes quanto à relevância, clareza e precisão dos itens. Os juízes validaram o instrumento segundo os valores, que permaneceram entre 0,88 e 1,0. Conclusões: o instrumento desenvolvido e validado é uma ferramenta capaz de garantir a segurança e padronizar a prática de imunização nas salas de vacina.

ABSTRACT

Se presenta un caso de hemangioma intramuscular con flebolitos de lengua en una mujer de 60 años. Clínicamente correspondía a una tumefacción de dos años de evolución, sin signos que sugirieran su origen vascular. Luego de su evaluación inicial se realizó biopsia excéresis de la lesión para su estudio histopatológico. En el estudio histológico se pudo observar una formación tisular redondeada y circunscripta constituida por tejido conjuntivo fibroso con abundantes capilares congestivos que desplazaban el tejido muscular, destacándose la presencia de trombos con calcificaciones dispuestas en capas concéntricas (flebolitos). En base a estos hallazgos morfológicos, el diagnóstico definitivo fue de hemangioma capilar intramuscular con flebolitos de lengua. Para confirmar la naturaleza neoplásica vascular de la lesión se realizó inmunomarcación con el anticuerpo GLUT1. Es de destacar, de acuerdo a la revisión bibliográfica realizada en PubMed, que esta entidad patológica es rara, particularmente en la lengua.


Apresentamos um caso de hemangioma intramuscular com flebólitos de língua em uma mulher de 60 anos. Clinicamente, correspondia a um inchaço com duração de dois anos, sem sinais que sugerissem sua origem vascular. Após sua avaliação inicial, foi realizada biópsia excisional da lesão para estudo histopatológico. No estudo histológico pôde-se observar uma formação tecidual arredondada e circunscrita constituída por tecido conjuntivo fibroso com abundantes capilares congestivos que deslocavam o tecido muscular, destacando a presença de trombos com calcificações dispostos em camadas concêntricas (flebólitos). Com base nesses achados morfológicos, o diagnóstico definitivo foi de hemangioma capilar intramuscular com flebólitos de língua. Para confirmar a natureza neoplásica vascular da lesão, foi realizada imunocoloração com o anticorpo GLUT1. Ressalta-se, de acordo com a revisão bibliográfica realizada no PubMed, que esta entidade patológica é rara, principalmente na língua.


Summary We present a case of intramuscular hemangioma with tongue phleboliths in a 60-year-old woman. Clinically it corresponded to a swelling of two years of evolution, without signs that suggested its vascular origin. After her initial evaluation, an excisional biopsy of the lesion was performed for histopathological study. In the histological study it was possible to observe a rounded and circumscribed tissue formation made up of fibrous connective tissue with abundant congestive capillaries that displaced the muscle tissue, highlighting the presence of thrombi with calcifications arranged in concentric layers (phleboliths). Based on these morphological findings, the definitive diagnosis was intramuscular capillary hemangioma with tongue phleboliths. To confirm the vascular neoplastic nature of the lesion, histochemical immunostaining was performed with the GLUT1 antibody. It is noteworthy, according to the bibliographic review carried out in PubMed, that this pathological entity is rare, particularly in the tongue.

ABSTRACT

RESUMO Objetivo caracterizar os vídeos que contém a demonstração do procedimento de administração de medicamentos por via intramuscular em indivíduos adultos. Métodos estudo de corte transversal descritivo, foram selecionados 44 vídeos brasileiros disponíveis no YouTube® que abordavam o procedimento de administração de medicamentos por via intramuscular. Resultados a maioria dos vídeos selecionados é de origem pessoal (86,4%), tem como autor um técnico de Enfermagem (59,1%), adota a região dorsoglútea como local de injeção (54,5%), foi produzido em ambiente de saúde utilizando um paciente para a demonstração do procedimento (52,3%). Nenhum vídeo apresentou a completude do procedimento, sendo identificada grande frequência de erros em todas as etapas do procedimento. Observou-se uma diferença estatisticamente significante entre os testes de confiabilidade e popularidade dos vídeos (p=0,042). Conclusão os vídeos que abordam o procedimento de administração de medicamentos por via intramuscular disponíveis na plataforma YouTube® foram considerados atuais, de pouca relevância, elaborados por fontes não confiáveis, de baixa acurácia e frágil finalidade. Contribuições para a prática os vídeos disponíveis na plataforma YouTube® sobre a administração de medicamentos por via intramuscular não devem ser indicados como material educativo para a formação ou atualização profissional.


ABSTRACT Objective to characterize videos that contain a demonstration of the procedure for administering drugs intramuscularly to adults. Methods a descriptive cross-sectional study, 44 Brazilian videos available on YouTube® were selected which addressed the procedure of intramuscular drug administration. Results the majority of the videos selected are of personal origin (86.4%), were made by a Nursing technician (59.1%), used the dorsal gluteal region as the injection spot (54.5%), and were produced in a healthcare environment using a patient to demonstrate the procedure (52.3%). No video showed the completeness of the procedure, and a high frequency of errors was identified at all stages of the procedure. There was a statistically significant difference between the reliability and popularity tests of the videos (p=0.042). Conclusion the videos on intramuscular drug administration available on the YouTube® platform were considered to be up-to-date, of little relevance, produced by unreliable sources, of low accuracy, and with a weak purpose. Contributions to practice the videos available on the YouTube® platform on intramuscular drug administration should not be used as educational material for professional training or updating.

ABSTRACT Objective: to create and validate an educational video on intramuscular drug administration using the Z-track technique. Methods: the Delphi Technique was used to validate the script. PhDs in Nursing and PhDs in Social Communication with experience in the production of educational videos participated in the process. After editing, the video was validated by three nursing professors and assessed by students of the undergraduate nursing program at a public university. Results: the video was validated by the examiners with 100% agreement in three rounds for script validation and in two for video validation after editing. The duration of the video was 9 minutes. Conclusion: after validation by the examiners, students assessed the video and considered it suitable for learning. We This video is expected to aid in the training of nursing professionals and the enhancement of patient care.


RESUMEN Objetivo: construir y validar un vídeo educativo sobre la administración intramuscular de fármacos, utilizando la Técnica Z. Métodos: se utilizó la técnica Delphi para validar el guion. Participaron en el proceso dos doctores en enfermería y dos doctores en comunicación social con experiencia en la producción de vídeos educativos. Tras su edición, el vídeo fue validado por tres docentes de enfermería y evaluado por estudiantes del curso de licenciatura en enfermería de una universidad pública. Resultados: el vídeo fue validado por los expertos con un 100% de acuerdo en tres rondas para la validación del guion y dos para la validación del vídeo después de la edición. La duración del vídeo fue de 9 minutos. Conclusión: tras la validación por parte de los expertos, los estudiantes evaluaron el vídeo y lo consideraron adecuado para el aprendizaje. Se espera que este vídeo pueda contribuir a la formación de los profesionales de enfermería y mejorar la asistencia a los pacientes.

Métodos: utilizou-se a Técnica Delphi para a validação do roteiro. Participaram do processo doutores em enfermagem e doutores em comunicação social com experiência na produção de vídeos educativos. Após a edição, o vídeo foi validado por três docentes de enfermagem e avaliado por estudantes do Curso de Graduação em Enfermagem de uma universidade pública. Resultados: o vídeo foi validado pelos juízes com 100% de concordância ocorrida em três rodadas para validação do roteiro e duas para validação do vídeo após a edição. A duração do vídeo foi de 9 minutos. Conclusão: Após a validação pelos juízes, estudantes avaliaram o vídeo e o consideraram adequado para o aprendizado. Espera-se que esse vídeo possa contribuir para a formação do profissional de enfermagem e melhoria da assistência ao paciente.

Intramuscular lipoma of the chest wall is a rare finding. We present the case of a 51years male who presented to the hospital with mass on the left side of the chest which was gradually progressive in size. After re- evaluation swelling was round in shape, single with well-defined margin and rubbery consistency. Preoperative imaging HRCT thorax was done which suggested a well-defined round, hypodense, non-enhancing space occupying lesion (10.2x 8.6 x2.3cm) with volume of approximately 100ml noted in the muscle of the chest wall (pectoralis major to pectoralis minor) in the left side of the chest most likely lipoma. Histopathology revealed normal adipocytes with small eccentric nucleus.

Cysticercosis is a Parasitic disease caused by the larval form of the pork tapeworm Taenia solium. This disease is a major health concern especially in developing world including India but its incidence is underestimated. Not too many cases of isolated intramuscular cysticercosis have been reported in pediatric population. Here we report a case of 4 years old female child presented with swelling over left lower chest over 3-4 months, diagnosed with intramuscular cysticercosis by ultrasonography.

ABSTRACT

Cysticercosis is an endemic disease in multiple areas of the world, caused by ingestion of embryonated eggs of Taenia solium. We reported a case of a 66-year old diabetic male presenting with decrease of consciousness and “rice grain” calcification in bothhemithorax and neck soft tissue region, without findings of cysticercosis in computed tomography (CT) scan. Cysticercosis most commonly affects the central nervous system, causing neurocysticercosis(NCC), while solitary intramuscular cysticercosis without involvement of the central nervous system is rare. Treatment of cysticercosis is indicated for symptomatic patients, including NCCsurgery is indicated in patients with neurovascular compromise caused by growth of cyst. Recognizing the signs and symptomsof cysticercosis is crucial for diagnosis and treatment in endemic areas where Taenia soliumis prevalent.

Background - Intramuscular hemangiomas are benign vascular tumours that most frequently occur in the trunk and extremities.They constitute about <1% of all hemangiomas, only 10%–20% have been found to arise in the head and neck region, and 36% of cases involving masseter muscle.They usually present as gradually enlarging soft-tissue mass with or without pain. They are located deep within the muscle, hence rarely exhibit clinical signs or symptoms such as bruits, thrills, pulsation which are suggestive of vascular origin. Due to their rare occurrence, unique location they are often confused with other soft-tissue pathologies such as parotid swelling or other muscular lesions. Case report - 21 year old male came with complaints of swelling and pain over the left cheek for past 6 months which was insidious in onset and non progressive.Patient did not have any history of trauma or increased salivation.On examination swelling of about 2*2 cm over the left cheek,well defined,non tender,soft in consistency.There was no pulsation,bruits or palpable thrill.There was no compressibility and the overlying skin was normal.There was no facial nerve involvement and parotid duct orifice was normal. Conclusion - The possibility of an Intramuscular hemangioma of the masseter muscle should be included in the differential diagnosis of any intra-masseteric lesion. Appropriate radiologic examinations, especially MRI, can enhance accurate preoperative diagnosis; the treatment of choice should be individualized in view of the clinical status of the patient.

ABSTRACT

Background: Theory classes in pharmacology are taking care of teaching of various oral and parenteral routes of drug administration, mainly focusing on knowledge domain but soft and psychomotor skills are under taught or deficient in undergraduate students. Demonstrating correct method of intramuscular route of drug administration and communication skills as introduced by competency based medical education (CBME) competency-based curriculum, will help in early acquisition of the skills and minimize the errors in administration. Aim and Objectives: The aim of the study is to teach the IM drug administration and communication skills to undergraduate students in pharmacology and to evaluate the perception of students and teachers toward new CBME curriculum. Materials and Methods: Correct technique of intramuscular drug administration was demonstrated on mannequins after going through the videos of IM. Students were told to perform it independently on mannequins. Perception of students and staff members were collected with pre-validated questionnaire provided after the practical teaching hour. Results: In our study, the majority of students (96%) felt that learning IM route of drug administration correctly will help in managing the patients in their near future days. They were well advanced in communicating with patients regarding usage of medications and were (86%) also in favor of introduction of this experiment in UG curriculum. Conclusions: Our study concluded that IM teaching technique introduced by CBME curriculum in practical classes was well accepted by students and were more confident in soft skills and psychomotor skills at the end of the study.

ABSTRACT

Objective:To investigate the MRI and pathological features of intramuscular fibro-adipose vascular anomaly (FAVA).Methods:The clinical and imaging data of 44 patients with intramuscular FAVA confirmed by pathology from December 2012 to March 2021 in Henan Province People′s Hospital were retrospectively analyzed. Twenty-five females and 19 males were included, with the age of (15±6), from 5 to 29 years old. The clinical and MRI features including the type, location, boundary, signal intensity, enhancement mode and degree, and the vascular flow voids in the lesion were summarized and compared with pathological results.Results:The thigh and calf muscles were involved in 1 patient simultaneously, and 1 site was involved in 43 patients, including 20 calf muscles, 15 thigh muscles, 5 forearm muscles, 1 upper arm muscle, 1 gluteal muscle, and 1 shoulder muscle. The gastrocnemius muscle of lower leg was most commonly involved (13/44), followed by soleus muscle (10/44) and quadriceps femoris muscle (9/44). All the lesions were solid on MRI, including 24 cases of focal mass type, 15 cases of diffuse infiltration type and 5 cases of local infiltration type. The long axis of all the lesions were consistent with the long axis of the muscles. All lesions showed inhomogeneously moderate hyperintensity on T 1WI and T 2WI, and significantly hyperintensity on fat suppression T 2WI. All lesions showed tortuous and dilated abnormal vessels, of which 18 cases showed vascular flow voids. Thrombosis was found in 10 cases. On contrast-enhanced imaging, the lesions showed moderate to obvious inhomogeneous enhancement. Pathologically, the diseased skeletal muscle was infiltrated by fibrous tissue, fat components, irregular abnormal veins and vessels, which led to inhomogeneous MRI signals. Among the 7 patients who underwent human PIK3CA gene mutation detection, and 6 were mutant. Conclusions:Intramuscular FAVA has certain characteristics in clinic, MRI imaging and histopathology, and its MRI signal characteristics can reflect its complex pathological components.

ABSTRACT Objectives: to compare adverse events after administrating hepatitis A vaccine intramuscularly in the ventro-gluteal region between techniques with and without aspiration. Methods: randomized double-blind clinical trial, using hepatitis A vaccine (inactivated) in the ventro-gluteal region, with a sample of 74 participants in the intervention group, vaccinated with the slow injection technique without aspiration, and 74 participants in the control group undergoing slow injection with aspiration. Daily assessment of participants was carried out in the 72 hours after vaccination, in order to ascertain local, systemic adverse events, local and contralateral temperatures. Results: the occurrence of local and systemic adverse events was homogeneous between the groups in the three days after vaccination (p>0.05). There was no influence of sex, race, pre-existing disease and use of medication. Conclusions: the intramuscular vaccination technique without aspiration in the ventro-gluteal region is safe for adverse events following immunization compared to the conventional technique with aspiration.


RESUMEN Objetivos: comparar eventos adversos después de la administración de la vacuna contra la hepatitis A por vía intramuscular en la región ventroglútea entre técnicas con y sin aspiración. Métodos: ensayo clínico aleatorizado doble ciego utilizando la vacuna Hepatitis A (inactivada) en la región ventroglútea, con una muestra de 74 participantes en el grupo de intervención, vacunados con la técnica de inyección lenta sin aspiración, y 74 participantes del grupo control sometidos a inyección con aspiración. La evaluación diaria de los participantes se llevó a cabo en las 72 horas posteriores a la vacunación, con el fin de conocer los eventos adversos locales, sistémicos, las temperaturas locales y contralaterales. Resultados: la ocurrencia de eventos adversos locales y sistémicos fue homogénea entre los grupos en los tres días posteriores a la vacunación (p>0.05). No hubo influencia de las variables sexo, raza, enfermedad preexistente y uso de medicación. Conclusión: la técnica de vacunación intramuscular sin aspiración en la región ventroglútea es segura para eventos adversos después de la vacunación en comparación con la técnica convencional con aspiración.

Foi realizada avaliação diária dos participantes nas 72 horas pós-vacinação, com intuito de averiguar eventos adversos locais, sistêmicos, temperaturas locais e contralaterais. Resultados: a ocorrência de eventos adversos locais e sistêmicos foi homogênea entre os grupos nos três dias pós-vacinação (p>0,05). Não houve influência das variáveis sexo, raça, doença pré-existente e uso de medicamento. Conclusões: a técnica de vacinação intramuscular sem aspiração na região ventro-glútea é segura quanto aos eventos adversos pós-vacinação em comparação à técnica convencional com aspiração.

SUMMARY: This study aimed to accurately localize the location and depth of the centre of the highest region of muscle spindle abundance (CHRMSA) of the triceps brachii muscle. Twenty-four adult cadavers were placed in the prone position. The curve connecting the acromion and lateral epicondyle of the humerus close to the skin was designed as the longitudinal reference line (L), and the curve connecting the lateral and the medial epicondyle of the humerus was designed as the horizontal reference line (H). Sihler's staining was used to visualize the dense intramuscular nerve region of the triceps brachii muscle. The abundance of muscle spindle was calculated after hematoxylin and eosin stain. CHRMSA was labelled by barium sulphate, and spiral computed tomography scanning and three- dimensional reconstruction were performed. Using the Syngo system, the projection points of CHRMSA on the posterior and anterior arm surface (P and P' points), the position of P points projected to the L and H lines (PL and PH points), and the depth of CHRMSA were determined. The PL of the CHRMSA of the long, medial, and lateral heads of the triceps brachii muscle were located at 34.83 %, 75.63 %, and 63.93 % of the L line, respectively, and the PH was located at 63.46 %, 69.62 %, and 56.07 % of the H line, respectively. In addition, the depth was located at 34.73 %, 35.48 %, and 35.85 % of the PP' line, respectively. These percentage values are all the means. These body surface locations and depths are suggested to be the optimal blocking targets for botulinum toxin A in the treatment of triceps brachii muscle spasticity.


RESUMEN: Este estudio tuvo como objetivo localizar con precisión la ubicación y la profundidad del centro de la región más alta del huso muscular (CHRMSA) del músculo tríceps braquial. Se colocaron veinticuatro cadáveres adultos en posición prona y se designó la curva que conecta el acromion y el epicóndilo lateral del húmero cerca de la piel como la línea de referencia longitudinal (L), y la curva que conecta los epicóndilos lateral y medial del húmero fue designada como la línea de referencia horizontal (H). Se usó la tinción de Sihler para visualizar la región nerviosa intramuscular densa del músculo tríceps braquial. La abundancia de huso muscular se calculó después de la tinción con hematoxilina y eosina. CHRMSA se marcó con sulfato de bario y se realizó una tomografía computarizada espiral y una reconstrucción tridimensional. Usando el sistema Syngo, fueron determinados los puntos de proyección de CHRMSA en la superficie posterior y anterior del brazo (puntos P y P'), la posición de los puntos P pro- yectados en las líneas L y H (puntos PL y PH) y la profundidad de CHRMSA. Los PL de la CHRMSA de las cabezas larga, medial y lateral del músculo tríceps braquial se ubicaron en el 34,83 %, 75,63 % y 63,93 % de la línea L, respectivamente, y el PH se ubicó en el 63,46 %, 69,62 %, y 56,07 % de la línea H, respectivamente. La profundidad se ubicó en el 34,73 %, 35,48 % y 35,85 % de la línea PP', respectivamente. Estos valores porcentuales son todas las medias. Se sugiere que estas ubicaciones y profundidades de la superficie corporal son los objetivos de bloqueo óptimos para la toxina botulínica A en el tratamiento de la espasticidad del músculo tríceps braquial.

ABSTRACT

Objective:To observe the clinical effect of modified Huiyangyin Decoction combined with Kinesio taping in the treatment of early and middle stage knee osteoarthritis.Methods:A total of 92 patients with early and middle stages of knee osteoarthritis, who admitted to our hospital from July 2021 to January 2022), were randomized into the western medicine group and the observation group by the random number table method, 46 in each group. The western medicine group received celecoxib capsules and sodium hyaluronate injection therapy, whicle the patients in the observation group received modified Huiyangyin Decoction combined with Kinesio taping on the basis of wetern medicine treatment. The treatment effect rates, angle of the knee joint initiative activity (AROM) changes and TCM syndrome (knee joint ache, swelling in the knee joint, rigid and claudication) changes, changes of pain intensity by visual analog scores (VAS), WOMAC scores, circumference of knee joint and Stand up time test, and adverse events were recorded and compared.Results:The total effective rate was 95.7% (44/46) in the observation group and 82.6% (38/46) in the control group, and the difference between the two groups was statistically significant ( χ2=4.04, P=0.045). After treatment, the flexion, internal rotation and external rotation of the observation group were significantly higher than those in the control group ( t=2.85, 2.29, 2.22, respectively, all Ps <0.05). The scores and total scores of knee pain, knee swelling, stiffness, claudication in the observation group were significantly lower than those in the control group ( t=20.53, 19.74, 17.40, 17.63, 27.13, respectively, all Ps<0.001). The VAS and WOMAC scores were significantly lower than those in the control group ( t=10.47, 10.55, respectively, all Ps<0.001). After treatment, the circumference of knee joint in the observation group [(33.69±1.11) cm vs. (34.55±1.37) cm; t=10.47, P<0.01] was significantly lower than that of the control group, and the time of standing up and walking was [(19.50±2.45) s vs. (28.12±2.22) s; t=10.55, P<0.01] were significantly shorter than those in the control group. During treatment, the incidence of adverse events in the observation group was 2.2% (1/46), and that in the control group was 4.3% (2/46). There was no significant difference between the two groups ( χ2=0.35, P=0.557). Conclusion:The modified Huiyangyin Decoction combined with Kinesio taping treatment has significant clinical effect, less adverse events for the patients with early and middle stage knee osteoarthritis.
